The Case Manager is responsible for the care coordination of their designated assignment, discharge planning, DRG Management, utilization review and quality improvement activities for their assigned patient population. Active participant in the multidisciplinary team discussing patient progress, barriers to discharge, recommending appropriate level of care internally and at time of discharge while proactively getting authorizations for durable medical equipment. The Case Manager completes a through screening assessment on all assigned patients. The case manager acts as a liaison/facilitator/advocate for the patient and/or family as needed. Ability to exercise good judgment in all situations. Commitment to upholding and providing services in a manner which is congruent with the Medical Center’s mission statement. Good organizational skills. Flexibility.
-Graduated from an accredited nursing program. Bachelor’s degree in nursing or related health care field preferred.
-Must possess current State of California licensure as a Registered Nurse
-Valid BLS card from the American Heart Association
-3 to 5 years of hospital experience strongly preferred,
-Case Management experience strongly preferred
– Ability to understand and speak English fluently to be easily understood by patients, patients families, hospital personnel, and physicians
